Clinic Information

Medication Updates

All medication must have a signed doctor’s order for us to administer it to your child at school. This includes all over-the-counter medication including cough drops. Additionally, all medication must be brought to the clinic and signed in by the student’s parent. Students may not carry any medication to or from school.

Please email the Schultz Clinic or look on the website for a copy of the required forms.

 Birthday Snacks

All birthday snacks for students must be approved through the clinic. We ask for 2 weeks’ notice so that we can ensure there are not allergies in the classroom and give those students with allergies the opportunity to bring an alternative snack. All snacks must be store-bought and individually wrapped.

Lunch and Smart Tags

The district purchased the first lanyard, cards and plastic cover for every student. Parents will be required to pay for replacement cards if they are broken, lost, or stolen. Every student is required to have their Lunch and SMART Tags. Parents will need to pay for the replacement via School Cash Online. A link will be emailed to you. Once payment has been received the front office will provide the replacement.
